Responsibilities -Social Services: -

planning and implementing a program that offers counseling and
other support services for residents and their families
evaluating, determining and creating a plan base on the
medical-related social needs of the residents
coordinating and implementing family meetings for residents to keep
the communicatin flowing
acting as the liasion between all parties
coordinating discharge planning

Qualifications -For Social Services: -
High School Diploma or equivalent -
LSW/MSW
Previous LTC/SNF experience preferred
Serious applicants -
