Job Details Full-timeEstimated: $85,000 - $110,000 a year3 hours ago Qualifications - Documentation review - PALS Certification - Clinical trials Full Job Description Category: Pharmacy Facility: Dayton Children's - Main Campus Department: Clinical Pharmacy Schedule: Full time Hours: 40 Job Details: Pharmacist -clinical generalist: Quality Improvement & Investigational Drugs Services Clinical generalist provides comprehensive pharmaceutical care through integrated drug distribution and clinical services. The pharmacist is responsible for the provision of safe, effective, and cost-efficient medication therapy to patients at Dayton Children's Hospital. Utilizing a decentralized pharmacy care model the pharmacist collaborates with the patient care team to monitor effectiveness of patient therapy, provides drug information to healthcare professionals, pediatric patients and their families, reviews physician orders, and coordinates dispensing of medications. The pharmacist oversees the daily pharmacy operations within their designated department to ensure the safe and appropriate delivery of medications to the patient. The pharmacist oversees the pharmacy students and technicians, and serves as the verification for products prepared by the technicians. The pharmacist adheres to State Board of Pharmacy laws and that of other regulatory agencies. As the Investigational drug services pharmacist - responsible for the inventory management of investigation drugs, all investigational record keeping, reviews protocols for investigation drugs and participates on the IRB and all investigational drug audits Investigational Drugs Services Responsibilities Reviews protocols for clinical investigational drug trials, as required. Liaises with principal investigators and/or trial coordinators/research assistants to establish pharmacy's role, determine fee schedules and to implement dispensing, compounding and billing procedures. Communicates with, and trains staff anticipated to participate in any aspect of the clinical investigational drug trial. Maintains a pharmacy binder which contains a study summary, protocol, dispensing procedures, completed samples of required paperwork, dispensing checklist, fee schedule, billing procedures and any other relevant materials for each trial. Maintains and coordinates distribution of an up-to-date clinical investigational drug trials list, as scheduled. Maintains responsibility for the management of the inventory for clinical investigational drug trials; orders, replaces and returns study materials, as required. Participates on the IRB and communicates any concerns regarding ethical issues or deviation from established policies & procedures Assists in ensuring the participation of the pharmacy department in all clinical trials which involve medications. Quality Improvement Responsibilities Responsible for identifying, documenting, participating in and leading quality improvement initiatives to improve patient care. Collect, research, investigate and present data and information for various reports and projects (i.e. controlled substance audits) Analyzes and evaluates the effectiveness of project activities for results and outcomes related to strategic goals (i.e. audit new processes). Report, as determined necessary, to appropriate committee on progress of specific projects. Supports organization to meet regulatory and accreditation standards (i.e. Joint Commission readiness) Medication Order Review Accurately verifies or enters medication orders into computer system, checks medication for correct drug, route, dose, patient, and frequency. Calculates dosage based on mg/kg and verifies patient allergy information prior to dispensing medications. Utilizes proper aseptic technique for sterile products. Follows guidelines for chemotherapy precautions, and prepares chemotherapy products. Intervenes with physicians when medication order is not appropriate for the patient. Documents intervention in computer system, and writes clarification slip to establish a change in the medication order. Dispenses medication safely and accurately for the patient. Appropriately labels all medications prior to dispensing. Prioritizes orders received in pharmacy based on clinical status of the patient. Dispenses medications in a timely manner, and adheres to department turnaround times for routine, ASAP, and STAT orders. Functions in the role of charge pharmacist when scheduled, and delegates tasks to others on the shift. Functions in the role of hematology/oncology satellite pharmacist, in charge of processing all orders for chemotherapy and prepares medication. Checks Medications Prior to Dispensing Accurately checks medications prepared by the technician for correct drug and dosage prepared. Follows up with checking medications to ensure efficient pharmacy workflow. Checks compounded medications and unit dose manufacturing prepared by the technician for accuracy and pharmaceutical appropriateness Monitors the activities of the technician to ensure that products are prepared safely and accurately. Ensures the work area for preparing medications is clean, disinfected, and free of unrelated supplies and equipment. Checks medications prior to being refilled in the Pyxis Medstations. Obtains controlled substances medications for technician to fill in pyxis. Monitors technician activity in Pyxis to ensure accuracy of filling medication. Checks drug bags used throughout hospital for appropriate inventory and integrity. (examples include EMS bags, urgent care box, and intubation kits). Ensures that emergency drugs are maintained and readily available during shift. Clinical Skills Attends Code Blue situations, and prepares medications for patient under direct supervision of physician. Keeps up to date with current PALS modules. Attends patient care rounds Attends educational programs and reads material relevant to pharmacy and health care to keep abreast of current information. Maintains continuing pharmaceutical education requirements as mandated by the State Board of Pharmacy Consults with providers regarding pharmacokinetic dosing, total parenteral nutrition parameters, continuous infusion calculations, and other patient care activities Knowledgeable of the drug information resources available: including LexiComp and Micromedex. Provides accurate drug information to health care professionals, pediatric patients and their families. Makes clinical decisions based on drug information provided.
