Job Description:
Summary/Objective: Primary job responsibilities involve service
linkage and case management to assist individuals in addressing
their behavioral health needs and tracking relevant health
outcomes. Case Managers are primarily community-based, providing
community support, case management, and service linkage both within
Boundless and with outside providers that identify and address
behavioral health needs, developmental issues, and barriers to
care. Case Managers will work with individuals, parents/guardians,
Boundless staff, and outside providers that support treatment and
contribute meaningfully to the whole health outcome of the
individual served. Essential Functions: Reasonable accommodations
may be made to enable individuals with disabilities to perform the
essential functions. Provides case management support to
individuals engaged in behavioral health services including
community psychiatric support and therapeutic support to promote
the coordination and implementation of treatment goals. Serve as a
liaison between families and agencies to positively promote
collaborative processes in the treatment planning for clients by
encouraging parent/guardian participation in treatment. Identify
and assess psychosocial and life skills needs through analysis of
factors impinging on client success including factors in the home,
school, and community. Provide parent/guardian educational
workshops on identified issues related to child development, stress
reduction, discipline and safety, and teacher/parent/client
communication. Keep caseload spreadsheet (information management)
updated regularly. Manage phone calls from families, guardians,
agencies, etc. Set up meetings; obtain information, etc. Develop
programs to address parent/guardian participation in agency
services and client engagement in the treatment process. Attend
meetings, training, and professional development activities as
required. Completes mandatory documentation within timelines
established by applicable regulations and policies. Mediates issues
that arise among clients, families, and staff. Provides linkage
with appropriate community resources by maintaining knowledge of
other systems, making referrals, and identifying unmet needs (e.g.,
recreational, transportation, healthy/safety, employment, etc.).
Provides clients and families with crisis management services as
needed. Work closely with an interdisciplinary team to support
clients and coordinate care. Develops a comprehensive discharge
plan in collaboration with clients, clients’ families,
multidisciplinary staff, and referral agencies. Trains and consults
with staff regarding psychosocial needs that encompass mental
health, development, and diversity of individual clients to
increase staff knowledge and skills. Advocates on a case, policy,
and program level. Meet productivity requirements as established by
management. All other assigned duties. Skills, Knowledge &
Abilities: Exemplary attendance. Excellent communication skills,
both oral and written, demonstrating professionalism with
co-workers, parents, and individuals. Excellent organizational
skills with the ability to stay focused. Ability to prioritize
multiple tasks. Ability to follow all company policies and
procedures. Demonstrated clinical knowledge related to behavioral
health issues and developmental disabilities. Intermediate computer
skills. Professional telephone etiquette. Must be able to show tact
and diplomacy when dealing with parents, clients, peers, and staff.
Education/Experience: Experience working with people with mental
health conditions. High School Diploma or equivalent. Preferred
Experience and Education: Bachelor’s degree in social work,
education, psychology, or another related field or at least three
years working in the human services field. Two years of experience
in a case management or care coordination role. Experience working
with people with autism spectrum disorders and developmental
disabilities. Additional Eligibility Qualifications: Valid Ohio
Driver's License with Ohio Bureau of Motor Vehicles; proof of auto
insurance; insurable driving record (five points or fewer). Ability
to obtain and maintain First Aid, CPR - adult, child, and infant.
(Training will be provided, if not already certified). Supervisory
Responsibility: This role does not require any supervision of
staff. Work Environment: This job regularly works in the community,
providing services outside of the office in community settings.
This role routinely uses standard office equipment such as
computers, phones, and photocopiers. Physical Demands: The physical
demands described here are representative of those that an employee
must meet to perform the essential functions of this job
successfully. While performing the duties of this job, the employee
is regularly required to talk or hear. The employee frequently is
required to stand, walk, and reach with hands and arms. Position
Type/Expected Hours of Work: This is a full-time position, and
hours of work and days are Monday through Friday, during business
hours. Occasional evening and weekend work may be required as job
duties demand. Travel: Travel is primarily local during the
business day, although some out-of-the-area and overnight travel
